Schizophrenia Test
A brief psychosis-risk screen adapted from the Prodromal Questionnaire (PQ-16). Sixteen questions covering unusual perceptions, paranoid thinking, and disorganized thought. This schizophrenia test flags whether these experiences are worth discussing with a professional.

1. I hear things that other people cannot hear like noises, or the voices of other people whispering or talking.
2. I have heard my own thoughts being spoken out loud.
3. My thoughts are sometimes so strong that I can almost hear them.
4. I sometimes see things that other people cannot see.
5. I have sometimes felt that other people are watching me or talking about me.
6. I have had the feeling that my thoughts were somehow broadcast so that other people could hear them.
7. I have felt as if my thoughts were not my own, but were put into my mind by some outside force.
8. I have felt that there were odd or unusual things going on that I could not explain.
9. I have felt that I could not trust other people because they were out to get me.
10. I have felt that my mind was somehow being controlled by an outside force.
11. I have sometimes felt as if things around me were not real or were like a dream.
12. I have sometimes felt that I was not in control of my own ideas or thoughts.
13. I often feel that others have it in for me.
14. I have been feeling unusually anxious or tense.
15. I often have difficulty concentrating or organizing my thoughts.
16. I am not interested in the things I used to be interested in.
About This Schizophrenia Test
This screener is adapted from the Prodromal Questionnaire-16 (PQ-16), developed by Ising et al. (2012). The PQ-16 screens for psychosis-like experiences that may indicate elevated risk for a psychotic disorder. It is not specific to schizophrenia alone. Positive screens can also relate to other psychotic spectrum conditions, severe anxiety, PTSD, or substance-related effects.
Schizophrenia affects approximately 1% of the global population. Early intervention during the prodromal phase significantly improves long-term outcomes. If you screen positive, a structured clinical assessment is the recommended next step.
